Dental pain is often severe, exhausting, and capable of significantly disrupting a person’s daily life. It typically occurs at the most inconvenient times—before travel, an important meeting, or during periods of high stress. In the practice of Dr Dimitar Chotorov, we understand how distressing acute dental conditions can be. For this reason, when an urgent examination is required, we aim to respond as quickly as possible.
In addition to emergency cases, oral surgical treatment is also an integral part of comprehensive treatment planning. It may include the extraction of non-restorable or compromised teeth, as well as surgical procedures involving the soft tissues and oral mucosa.

Aim of emergency treatment?
The primary aim of the emergency examination is to establish a rapid and accurate diagnosis and to relieve pain. Once the condition has been stabilised, a comprehensive treatment plan is developed in a planned manner to restore function, oral health, and the aesthetics of the dentition.
When does the condition require hospital treatment, and is it performed under anaesthesia?
Surgical procedures are performed under local anaesthesia (the patient remains conscious) and within the competence of the treating team. In cases requiring highly specialised treatment, hospital settings, or intervention by an oral or maxillofacial surgeon, patients are referred to the appropriate specialist following examination and assessment of the condition.
When is a painful tooth extracted?
Patients need to understand that not every painful tooth requires extraction. Indications and contraindications for tooth removal are numerous and require comprehensive diagnostics, followed by an individual treatment plan.
Pain alone is rarely a sufficient reason for extraction, and in many cases, the tooth can be successfully treated using significantly more conservative methods. Even in emergencies with severe pain, a correct diagnosis remains essential, and extraction is always considered the last therapeutic option.
